as identity theft, the Patriot Act requires all financial
institutions to obtain, verify and record information that
identifies each person who opens a new account or requests
a loan.
What does this mean to you?

name, address, date of birth, taxpayer identification number
and other information that will allow us to identify you.
• We may also ask to see you driver's license or other
identifying documents.
In some cases, identifications may be requested from existing
customers of Harwood State Bank if the original documentation
